This guide will show you where to find every collectible in Fatal Frame / Project Zero 4: Mask Of The Lunar Eclipse. Here you will find every Note, Diary, Spirtit Radio Recording, Cassette Tape, every type of ghots (Revenant, Specter, Wraith), every Hozuki Doll, every Lens, every Upgrade for the camera and all Blue and Red stones to upgrade the camera. Almost everything is missable. For most collectibles you will only get 1 shot at, so I recommend you make a save for every chapter. This way, if you miss something you can just load the chapter from the start. You can also just wait until your second playthrough since the collectibles will carry over.

Prelude
File - Nurse's Memo. Obtained automatically when you pick up the key in the Management Office(story related).
File - Professor Aso's Notes(1). Found in the same room as the Camera Obscura. You will have to pick this up before getting the camera.
Ghost (Wraith) - Yoriko Sonahara. You will have to defeat this wraith as soon as you pick up the camera for the first time.
Hozuki Doll. Under the desk in the corner of the room where you get the camera.
Phase I
File - Note Form Madoka (1). On the gate at the top of the stairs near the first save point of the game.
File - Article: "Islanders Found Dead". On the shelf to your left in the room you follow the ghost in to (same room where you get the camera).
File - Professor Aso's Notes (2). On the ground to the right of the previous file.
File - Note From Madoka (2). Obtained automatically when you pick up the camera.
File - Article: "Prof. Aso Arrives". On the board to the right of where you pick up the camera.
Blue Spirit Stone x3. On the ground to the right of the shelf where you got "Islanders Found Dead" article.
Ghost (Wraith) - Fuyuko Shiratsuki. You will get attacked by this wraith after leaving the room where you get the camera.
Blue Spirit Stone. In the area where you encounter the wraith. In front of the door of the room where you got the camera.
Hozuki Doll. Inbetween the curtains of the window to the left of the blue stone.
File - Rogetsu Isle Pamphlet. Return to the entrance hall (the first room with the save point) to find a new blue items on the ground near the desk. Pick it up to get the file.
Hozuki Doll. Behind the desk near the previous file.
Ghost (Revenant) - Nurse On Watch Duty. Look up to the windows above the desk through the camera until the circle turns blue. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.
Ghost (Specter) - Boy Running Away. Now, return to the hall where you found the doll behind the curtains and follow the hall to the end. You will soon get to an area with some stairs that lead up to the top floor. Here you will see a short cutscene of a boy running away. Quickly snap a photo of him to register the specter.
Ghost (Specter) - Man In Kitchen. Go through the door that the boy was running to. You will now be in a room with a piano. Here, pick up the Herbal Medicine on your left. As soon as you do you will see a specter appear. Quickly snap a photo.
Ghost (Specter) - Nurse Who Misplaced Key. From the previous specter, turn around and look through your camera to see this specter near the table.
File - Information Plaque. Obtained when you pick up the key from the table next to the specter in the piano room.
File - Note From Madoka (3). On the piano in the same room as the specter.
Lens - Blast Lens. Found on the ground behind the piano.
Hozuki Doll. Found inside the fireplace behind the proyector screen to the right of the piano.
Blue Spirt Stone x5. On the table in the same room as the piano.
Ghost (Specter) - Boy Wearing Mask. Go to the double door on the other side of the same room. To the right of the double door you will see some masks. Examine them and quickly turn around and take a photo of the specter.
Hozuki Doll. Go through the door near the mask frames. On the other side, you will find the doll on the shelf to your left.
Ghost (Specter) - Man In Courtyard. Continue down the hall until you get to a window. Here, your fillament indicator will turn yellow. Quickly look through the window to take a photo of the specter.
File - Article: "Last Survivor Dies". On the small box on the wall to the right of the window where you took the photo of the previous specter.
Ghost (Specter) - Boy Standing Beyond The Gate. Continue down thhe hall to return to the entrance room. Here you will see a short cutscene with a ghost behind the gate. Take a photo to register the specter.
Hozuki Doll. Go down the left side of the stairs to find a door. You can open this door with the key you got from the dining room. Inside one of the boxes you will find the doll.
File - Article: "Investigation Stalls". On the floor in the same room as the previous doll.
Voice - Hoarse Voice. After solving the puzzle to open the gate in the first room (use code 72103), examine the ringing phone on the front desk.
Ghost (Specter) - Nurse With Patient. From the phone, head up the stairs to see a small cutscene with 2 ghosts. Quickly take a photo of them.
Red Spirit Stone x5. At the top of the stairs where the specter was, go right to find the stones on the couch.
Hozuki Doll. Just past the stone, head in to the control room on your right. The doll will be under the desk.
Ghost (Specter) - Woman Greeting Ruka. Exit the control room and head through the door in front of you. Here, take a few steps fowards to find the specter.
File - Note From Madoka (4). Continue down the hall. After 2 corners you will find the note on the floor.
Ghost (Specter) - Boy Disappering Into Room. Take a few steps forward from the note to see the specter.
Hozuki Doll. Now, follow the specter in to room 206. Inside you will find the doll in a plant pot on your left.
Voice - Patient Interview: Himiko Kiriya. In the same room on the chair to the right of the doll.
Blue Spirit Stone. On the floor to the right of the previous collectible.
Boy's Diary (1). On the desk in the same room.
Ghost (Wraith) - Kazuto Amaki / Kazuto Amaki (Blossomed). Examine the mask near the bed and you will be attacked by this wraith.
NOTE: Some ghosts can become “Blossomed” in specific encounters. This is the state they enter when their face goes blurry. Getting them in to this state requires some patience, since it seems like it happens at random. This guide will mention the locations they blossomed for me.
Boys Diary (2). Obatined after defeating the wraith.
Hozuki Doll. Leave room 206 and you will see some shelves above you on the right side of the hall. The doll will be on one of these sleves.
Upgrade - Alarm. On the floor at the end of the same hall.
Ghost (Wraith) - Natsuki Shiono. You will face this wraith on the way back to the entrance hall.
Ghost (Specter) - Man Pressing Against The Window. Once you make it back to the entrance hall (the room with the save point), go through the door to the right of the stairs. After taking a few steps a short cutscene will play with a specter outside the window. Quickly take a photo to register the specter.
Ghost (Wraith) - Tadayuki Kaido. Continue to the dining room. Here, place the mask that you got from the boys room in to the empty frame. Now, return through the hall where you encountered the previous specter. Just keep walking down the hall until you get attacked by this ghost.
Red Spirit Stone x5. Now, go through the entrance hall to return to the hall where you got the doll behind the curtains. Go past the curtain and through the door. On the other side of the door the hall will bend. The stone will be on the ground on your right.
Red Spirit Stone x5. Continue through the hall until you come out to the room with stairs (same room where you encountered the first specter). Here, go up the stairs to find the stones on the couch in front of you.
Hozuki Doll. Return to the room where you got the camera. You will now be able to open the other door in the room (if you have placed the mask in the frame). On the other side of the door the doll will be in front of you on one of the bookshelves.
Ghost (Wraith) - Madoka Tsukimori / Wraith - Madoka Tsukimori (Blossomed). You will encounter this wraith automatically at the end of the level.
Phase II
File - Prof. Aso's Notes (3). Obtained automatically at the start of the level.
File - 2F Room Assignemts. Enter the control room (the small room straight in front of you at the top of the stairs). The file can be found on the desk to the left.
Blue Spirit Stone. On the desk to the right of the previous file.
Upgrade - Evade. On the floor in the same room as the previous collectibles.
File - 2F Nurse's Logbook (1). On the other desk in this same room.
Ghost (Specter) - Patient In Front Of Door. As soon as you exit the control room you will see a ghost. Take a photo to register the specter.
Ghost (Specter) - Girls Running Under Stairs. Now, go down the stairs to the entrance hall. As you go down the stairs you will see 2 girls. Quickly snap a photo of the girls as they run near the stairs (if they get to the door it will be too late and will count as the next specter).
Ghost (Specter) - Girls Looking Into Room. The same girls as before. For this photo you will have to wait for them to get to the door. To get both of the photos in one go can be tricky. If you fail you can just reload the save at the start of the chapter to try again.
Ghost (Specter) - Man Looking At Entrance. Now, head to the main entrance (the double door to the left of the clock). As soon as you get close to the door you will see the specter.
Red Spirit Stone. In the corner to the right of the main door.
File - Memo: "New Patient". Found on the floor near the desk in the same room as the previous collectibles.
Blue Spirit Stone x3. In the room under the stairs. On top of the box where you entered the code in the previous chapter.
Blue Spirit Stone x3. Head through the door to the south (the one to the right of the stairs). Go to the end of this hall to find the stones at the end.
Ghost (Specter) - Nurse Seeking Help. After getting the code from the clock, return to the control room upstairs. As soon as you open the door you will see the specter.
Ghost (Wraith) - Fuyuko Shiratsuki / Fuyuko Shiratsuki (Blossomed). As soon as you enter the code in to the intercom this ghost will attack you.
Note - Female Patient's Suicide Note. Exit the control room and go through the door in front of you. Go down the hall to find the note just after the first door.
Ghost (Revenant) - Woman Hanging Herself. After reading the previous note, look through the gap where the broken door is to see a noose. Snap a photo to see the ghost.
Ghost (Wraith) - Iori Aiba / Iori Aiba (Blossomed) & Natsuki Shiono. Return to the door that you walked past before you picked up the previous note. examine it to start the battle against the 2 ghosts.
Ghost (Specter) - Girl Standing On Stairs. After taking a photo of the door you examined before the encounter with the 2 ghosts, return to the entrance hall. As soon as you get down the stairs a ghost will appear at the top of the stairs behind you. Turn around and take a photo as quickly as possible.
Lens - Stun Lens. Now, go through the hall to the north (the one near the desk) and enter the first door on your left (you should now be in the room where you got the camera for the first time). Once inside, go through the door on the right. In this room look at the bookshelf to your right as soon as you go in to find the lens (this can take some time to appear so just keep looking at the bookshelf until it does).
Red Spirit Stone x5. On some books to the left of the bookshelf with the lens on it.
Ghost (Wraith) - Tadayuki Kaido. Walk up to the mirror in the same room as the previous collectibles to start the encounter with this wraith.
Ghost (Specter) - Woman Standing By Masks. After picking up the key from the mirror, go to the door to exit the room. As soon as you interact with the door the specter will appear.
File - Prof. Aso's Mask Inquiries. After taking the photo of the specter, walk up to where the ghost was standing to see this file and a mask piece in the glass showcase cabinet.
Ghost (Specter) - Collapsed Girl. Once you get back to the entrance hall you will see a ghost on the floor. Take a photo to register the specter.
Ghost (Specter) - Girl Looking Down From Stairs. A few seconds after taking the photo of the previous specter you will see this ghost at the top of the stairs.
Ghost (Wraith) - Fuyuko Shiratsuki. Go up the stairs and enter the control room again. This ghost will attack you when you get close to the intercom.
File - Madoka's Diary. After interacting with the intercom, exit the control room and enter the hall in front of you. You will now be able to open the first door on the right with the key. In the next room, you will find the diary on the table in front of you.
File - Letter To Madoka. In the same room as the previous collectible, on the desk to the right.
Upgrade - Measure. Under the desk where the previous collectible was.
Ghost (Specter) - Madoka Sitting On Bed. Enter the room to the left of the desk to see the ghost sitting on the bed.
Red Spirit Stone x5. On the dresser in the bedroom where the ghost it.
File - Nurse's Logbook (2). Interact with the ghost on the bed. She will leave the file behind.
Hozuki Doll. Inside the clock above the bed where the ghost was.
Ghost (Specter) - Girl Peeping At Mizaki. As soon as you leave the bedroom the ghost will appear behind you.
Ghost (Specter) - Nurse Being Called By Ayako. As soon as you exit back out to the hall you will see this ghost walking in front of you.
Ghost (Specter) - Girl Pulling Nurse. Follow the nurse around the corner to find this specter.
Ghost (Wraith) - Ayako. At the end of the hall enter the door on the left to enter the room where you encounter Ayako.
Phase III
Voice - Choshiro’s Investigation Notes (1). Obtained automatically at the start of the level.
Ghost (Wraith) - Katsuhito Kariya, Yoriko Sonohara / Yoriko Sonohara (Blooming) & Sanae Hozuki. You will encounter these 3 wraiths as soon as you enter the building.
Lens - Camera. Obtained after the triple ghost encounter. With this you will be able to take photos with the flashlight.
Red Spirit Stone x5. On one of the benches in the room where you figh the 3 ghosts.
File - “Warning: Wandering Patients”. Found on the reception desk in the same room.
Blue Spirit Stone. Head up the stairs to the left of the elevator. At the top go right to find the stone at the end of the hall.
File - Medical File: Asagi Hizuki. Now return to the stairs and enter the door straight on front of you (the one with the cracked window). You can find this file on the desk to your left.
Hozuki Doll. Turn around and look through the window in front of you. The doll can be found in the left bottom corner.
File - Haibara’s Assistant’s Notes (1). On the bookshelf to the right of the window.
Blue Spirit Stone. On the sink in this same room.
Ghost (Specter) - Long-Haired Girl. Will appear in front of you as soon as you enter the next room.
Red Crystal x5. On the bed to your left in the same room as the previous ghost.
File - Diary With Pictures (1). On the bed where the ghost was standing.
Ghost (Specter) - Assistant Wandering The Corridor. As soon as you pick up the previous file a the specter will appear on the other side of the window.
Hozuki Doll. Go down the hall where the previous specter was. The doll can be found in the wheelchair at the end.
Ghost (Specter) - Assistant In Smoking Area. Now go through the curtains to enter another hall. Walk forward until the ghost appears in front of you.
Ghost (Wraith) - Sanae Hozuki. After picking up the key and map from where the previous ghost was standing, head back through the previous room to get attacked by this wraith.
Ghost (Specter) - Man Standing By Window. Continue backtracking in to the next room. The specter will soon appear on the other side of the window to your left (remember to put the lens back on the flashlight after the previous wraith encounter).
Ghost (Specter) - Man Reflected In Mirror. Turn around and walk up to the mirror in the same room to see this specter.
Ghost (Specter) - Assistant Heading To OR. Now, return to the bottom floow. Walk towards the save point to see this specter walking down the hall.
Voice - Unknown Voice (1). Answer the phone on the reception desk in the same area as the save point.
Blue Spirit Stone x5. Now, go through the door at the end of the hall where the specter was heading. In the next hall, you will find the stones on the couch.
Hozuki Doll. Continue down the hall by going through the curtains. Here you will find a sink with herbal medicine on it. Above this sink you will find the doll.
File - Article: “Mass Disappearances”. From the sink, turn around and examine the lockers to get this file.
Red Spirit Stone x5. On the floor to the left of the lockers.
Ghost (Specter) - Doctors Performing Operation. Go through the door to the left of the lockers. Here, walk around the curtains and snap a photo of the 2 ghosts at the operating table in the middle of the room.
Blue Spirit Stone. Found on the table to the right of the sink in the corner of the same room as the ghosts.
Hozuki Doll. On the floor to the left of the sink in the corner.
Ghost (Wraith) - Asagi Hizuki. Examine the operation table in the center of the room to start the encounter with this wraith.
File - Diary With Pictures (2). Obtained after defeating Asagi Hizuki.
Ghost (Specter) - Assistant In Front Of Desk. Examine the operating table again to get the death certificate. Then, follow the ghost in to the next room. Here you will find the specter.
File - “What is the Rogtsu Kagura?”. As soon as you snap a photo of the ghost, this file will be on the shelf to the right.
File - Haibara’s Assistant’s Notes (2). After snaping the photo of the ghost, pick up the blue item on the shelf to your right. Then, go to where the ghost was standing to get this file.
Red Spirit Stone x5. To the left of the previous note.
Hozuki Doll. From the previous collectible, look to your right to find the doll inside the glass cabinet.
Ghost (Wraith) - Shoji Katagiri. Now start backtracking. Just before you leave the operating room you will get attacked by this ghost.
File - Informer’s Report. Obatined after defeating Shoji Katagiri.
Ghost (Wraith) - Asagi Hizuki & Sanae Hozuki. Continue backtracking and you will get attacked by these 2 ghosts in a hall.
Voice - Call From informer. Once you get back to the entrance hall, answer the phone to get this voice.
Ghost (Wraith) - Fuyuko Shiratsuki & Michihiko Ikushima. You will get attacked by these 2 ghosts after answering the phone.
Hozuki Doll. Now open the map and look for the door with the key icon on it (the one to the west of the reception desk). Open this door and head inside. The doll will be on your right as soon as you enter the room.
File - “Moonlight Syndrome And The Moon”. On the desk in this same room.
Voice - Patient Interview: Takashi Aiba. Obtained at the same time as the previous file.
Ghost (Specter) - Nurse Placing Wooden Card. On the other side of this room you have 2 doors. First, go through the one on the right. Once inside look to your left to find the specter.
Hozuki Doll. From the previous ghost, turn around to find the doll in the corner.
Ghost (Specter) - Assistant Hidding Something. After getting the item from where the previous specter was standing, head back out to the previous room. Now, go through the door in front of you. In this hall the specter will appear on the other side of the window.
File (Haibara’s Assistant’s Notes (3). On the desk in the room where the beeping is coming from.
Ghost (Wraith) - Shoji Katagiri. Interact with the intercom where the file was. You will then get attacked by the wraith.
Blue Spirit Stone x3. From the intercom, turn around and go through the curtain. On the other side you will find the stones.
Hozuki Doll. To the right of the stones.
Red Spirit Stone x5. After getting the engraved box from the other shelf in the room behind the curtains, return to the hall. Then, go through the door on the left to get to a new hall. Here, get the stones from the bed.
Ghost (Specter) -Assistant Behind Fence. Continue through down the stairs in the hall. At the bottom get close to the metal bars on the left to trigger a scene. The ghost will be on the other side of the metal bars after the scene.
File - Memo With Code. Pick up the item behind the metal bars where the specter was. With this item the box will open (you must have picked up the box in the room behind the curtains where the intercom was & the item where the “Nurse Placing Note” specter was) giving you the file.
Hozuki Doll. In the same area as the specter behind bars you will see a wheel chair covered with a white blanket. The doll can be found behind it.
Ghost (Specter) - Director Haibara Passing By. Go back up the stairs and enter the door on the right. This specter will be standing in front of you as soon as you walk in.
Blue Spirit Stone x3. In the same room on the shelf to the left.
Hozuki Doll. On the lower part of the bookshelf to the right of the previous collectible.
File - Director haibara’s Notes (1). From the doll, turn around to find this file on the shelf on the opposite side of the room.
NOTE: The code for the Director’s Office is 276.
Ghost (Wraith). Katsuhito Kariya /Katsuhito Kariya (Blossomed). After inserting the code in to the back of the desk in the directors office, examine the cabinet. This will reveal some stairs. At the bottom of the stairs you will be attacked by this wraith.
Blue Spirit Stone x5. Under the stairs in the same room as the wraith.
Hozuki Doll. In the box to the left of the stones.
File - Moonlight Syndrome Research File. On the desk to the rigth of the bed in the same room as the previous collectibles.
Blue Spirit Stone x3. On the table to the left of the bed.
Lens - Focus Lens. Examine the device on the bed and head down the stairs. At the bottom of the stairs look on the ground to your left to find the lens (this item can take a while to appear).
Phase IV
File - Note From Madoka (5). In the corner of the room you start in.
Blue Spirit Stone x10. In the corner to the left of the previous file.
Red Spirit Stone x5. After leaving the room you start in, you will now be in the Museum. You can find the stones on top of the sofa.
Ghost (Specter) - Nurse Heading To Station. Right in front of you as you leave the previous room.
Ghost (Specter) - Nurse Doing Rounds. Follow the specter through the door on the right. Once you get to the entrance hall you will see her going up the stairs. Take a photo to register this specter.
Ghost (Specter) - Nurse Called By Patient. Follow her up the stairs. Then, at the top go right to find her again at the entrance of the control room. Take another photo of her here to get this specter.
Ghost (Specter) - Nurse Operating Control Panel. Follow the nurse in to the control room to see here again. Take a photo of her here to register this specter.
File - 2F Nurse’s Logbook (3). Interact with the control panel where the ghost was and turn it off. You will get this file as soon as you do that.
Ghost (Wraith) - Fuyuko Shiratsuki. After answering the intercom this ghost will appear.
File - Memo: “Musical Therapy”. Obtained automatically after defeating the ghost.
Ghost (Revenant) - Woman Standing In Red Room. Leave the control room and go through the door in front of you. In this hall you will see a door that you can’t enter on your left. Look through the camera after interacting with it to register this revenant. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.
File - Madoka’s Notebook. Now, enter the door on your right. Go through to the 2nd room to find the file on the dresser to your right.
Ghost (Wraith) - Kazuto Amaki. Return to the hall and enter the next door on the left. Here there is a chance Kazuto Amaki (the boy you have encountered in the past) appears and fights you again (this is useful if you didn’t get him to blossom and what another go).
Ghost (Specter) - Ayako Beconing. Exit Kazuto’s room and continue down the hall. Enter the last door on the left (Ayako’s Room). After taking a few steps Ayako will appear. Take a photo of her to register the specter.
File - Medical File: Ayako. On the bed in Ayako’s Room.
Blue Spirit Stone. On the floor to the right of the bed.
Blue Spirit Stone x10. On the dresser at the end of the bed in Ayako’s room.
Hozuki Doll. On the opposite side of the room from the bed you will find a doll behind a screen. The collectible doll is behind this doll. You can get it by going to the right side of the screen.
Ghost (Wraith) - Ayako. Open the door behind where Ayako was standing. This will start the encounter against her.
File - Black Notebook (1). After going through the small door, look to your left to find the note on the rocking house.
Ghost (Specter) - Ayako In Bed. In the same room as the note, examine the side of the bed to get Ayako to appear. Then, take the photo.
Hozuki Doll. In the same room as the previous collectibles. You can find the doll on the wooden beam above the door you used to get to this room.
Voice - Madoka Begging For Help. Return to the entrance hall. As you are going down the stairs you will hear the phone ring. Answer it to get this voice file.
Ghost (Specter) - Woman In Black Lurking. Exit the entrance hall through the door to the south (the one to the right of the stairs). Start walking down the hall to see a woman in black standing on the other side of the window to your left.
Red Spirit Stone x5. Continue to the end of the hall to find the red stones.
Blue Spirit Stone x5. Go through the door on your left to enter the dining room. Here you will find the stone on the table in the middle of the room.
Ghost (Specter) - Woman In Black Walking. After playing the notes on the piano in the dining room, exit through the door to the north. Here you will see the specter come out of the lift.
Red Spirit Stone x5. On your left, after taking the photo of the specter.
File - Letter Dropped By Woman In Black. This note is dropped by the previous specter in front of the lift.
Ghost (Specter) - Woman In Black With Wheelchair. Go to the save point to get this specter to appear.
File - Information About Lift & Rogetsu Hall 1F/B1F Guide. Enter the lift that the specter used to get here. Then, examine the control panel to get the files.
Ghost (Specter) - Woman In Black Going Out. Now go back to the entrance hall (MAKE SURE YOU GO BACK THROUGH THE PIANO ROOM TO GET THERE). Here, go through the hall to the north to finde this specter (if you come from the other side it’s a lot harder to take the photo).
Ghost (Specter) - Woman In Wheelchair. Follow the ghosts through the door on the right. In the new area look left and take a photo of the ghost in the wheelchair.
Lens - Slow Lens. In the bushes to your right.
Blue Spirit Stone x5. Continue until you get to the wheelchair. Before interacting with it, turn around and go to the street lamp in the corner to find the stones.
Red Spirit Stone x5. In the same open area as the previous stones. These are near the building just before the stairs that go down.
Hozuki Doll. To the left of the pool you will see a window below some broken stairs. The doll can be seen inside.
Ghost (Wraith) - Kageri Sendo. Now, examine the wheelchair to start the encounter with the wraith.
Hozuki Doll. Now return to the elevator room (where you first see the ghost with the wheelchair). Here save the game and open the locked door by using the key you got near the pool. Inside, head half way down the stairs and look on the shelf to your right to find the doll.
Hozuki Doll. At the bottom of the stairs, go through the door on the right. You will now be in a small room. Here, look through the metal grate on the floor to find the doll under some sandbags.
File - On-Duty Nurse’s Memo. Now go back through to the previous area and go through the other door. In this room you will find the file on the desk.
Blue Spirit Stone x10. In the corner of the same room.
Hozuki Doll. Hanging on a pipe above the blue spirit stones.
Ghost (Specter) - Patrolling Man. As soon as you solve the puzzle in the same room as the previous collectibles (solution is UP, UP, DOWN, DOWN, UP), the specter will appear behind you.
Ghost (Specter) - Man Guarding Door. Follow the previous specter through the door. Continue through the hall until you have some stairs on your right. Look down the stairs to find the specter.
Hozuki Doll. On a wooden beam above the previous specter.
Lens - Pierce Lens. On the ground below the previous doll.
Ghost (Wraith) - Masanobu Fuchiki. try to head back through the door to the previous room. The door will now be locked and you will get attacked by this wraith.
File - “On The Court Of The Unhallowed”. Obtained automatically after defeating the wraith.
Ghost (Wraith) - Tomoko Hinuma / Tomoko Hinuma (Blossomed) & Tadayuki Kaido / Tadayuki Kaido (Blossom). Now go all the way back to the entrance hall. Here you will encounter the 2 wraiths.
Ghost (Specter) - Man In Lift. It’s now time to return to the lift where the wheelchair ghost first appeared. As soon as you step inside this specter will appear in front of you.
Hozuki Doll. Use the lift to go to 3F. Here, look left as you make your way down the stairs to find the doll behind the screen.
Ghost (Specter) - Nurse Waiting. Go back up the stairs and enter the door in front of you. Inside, you will find the specter by sticking to the left.
Hozuki Doll. When entering this room (the room with the specter) you will have a locker to your left. The doll can be found on top of it.
File - 3F Room Assignments. In the same room as the previous specter.
Ghost (Specter) - Nurse Heading Towards Room. After solving the puzzle (solution is 1956) the specter will appear behind you.
Red Spirit Stone x5. As you make your way to the door that the nurse goes through you can find the stones on the couch on the right.
Phase V
File - Ayako’s Note. On the bed in the room you start in.
Blue Spirit Stone x10. Also found on the same bed.
Ghost (Specter) - Girl Staring At Misaki. Examine the door to get this ghost to appear behind you.
File - Torn-Up Note. Go through the small door where the specter was standing. In this room you can find the note on the bed.
Red Spirit Stone x5. Examine the lamp on the wall in the bedroom to get the stones.
Ghost (Wraith) - Ayako /Ayako (Blossomed). Examine the door in the bedroom. Then, take a photo of it. Now, go back to the room you started in and examine the doll behind the screen to get the rusty key. Finall, return to the door in the bed room and try to open it to start the encounter against the wraith.
Lens - Slow Lens. After the wraith encounter, head through the door to the next room. Inside, you will find the lens on the jacket hanging on the wall to your left.
File - Rogetsu Kagura Film Record. On the bed in the same room as the previous collectible.
File - Black Notebook (2). On the same bed as the previous file.

Ghost (Revenant) - Head Specimen In OR. In the same room as the previous collectibles you will notice that the filament is blue. Look at the locked box on top of another box with books in it with the camera. As soon as the circle turns blue, take the photo.
Ghost (Revenant) - Blossoming Patient. Now, go through the door. You will be in the area with the stairs and lift. Here, stay on this floor and go through the door on the left of the stairs to reach the balcony. Here look down to the garden (specifically to where the wheelchair ghost was in the previous chapter) and take the photo as soon as the circle turns blue.
Blue Spirit Stone x3. Now return to the stairs and go down to the bottom floor (where the save point is). Here, you can find the stones on the couch.
Red Spirit Stone x5. Head to the dining room by going through the door to the south. Here you can find the red stones on the counter to the left.
Blue Spirit Stone x3. Head to the right as soon as you come in to the dining room to find the stones on the second table.
Ghost (Wraith) - Madoka Tsukimori. Examine and play the proyector in the dining room to start this encounter.
Note From Madoka (6). Obtained after defeating the ghost.
Ghost (Specter) - Man Sinking Into Corridor. Exit the dining room through the western door. Take a few steps in the hall for this specter to appear behind you.
Ghost (Specter) - Girl In Lift. Go back through the dining room to return to the lift. As soon as you walk in front of it the specter will appear inside the lift.
Ghost (Wraith) - Katsuhito Kariya. Take the lift up to 2F. You will encounter this ghost as soon as you get off the lift.
Blue Spirit Stone x10. Next, go through the hall to the left of the stairs and through the door at the end. The stones will be on the couch to your right.
Ghost (Specter) - Patients Standing In Entryway. Go down stairs to the entrance hall for these ghosts to appear at the main door.
Ghost (Specter) - Woman In Corridor. Go back up stairs and head right. Then, go through the door on your left. Continue through the hall until this specter appears behind you.
File - Torn Diary. Enter the door on your right to enter Makoto’s Room. Here you will find the file on the dresser near the bed.
Blue Spirit Stone x5. In the same room, on the other dresser.
Voice - Young Madoka’s Voice. Continue down the hall and enter the door on the left (Room 206 - Kazuto Amaki’s room). Here examine the spirit radio to the left of the bed to get this voice file.
Ghost (Revenant) - Squatting Boy. Turn around and look at the papers on the floor to the left of the door through the camera. As soon as the circle turns blue, take the photo.
Blue Spirit Stone x15. Return to the lift and go to 3F. As soon as you get off the lift, look to your rigth to find the stones on the couch.
Red Spirit Stone x5. Go down the first set of stairs to find these stones in the corner.
Red Spirit Stone x5. Head back up the stairs and go through the door to the south. On the other side of the door you will find the stones to your left.
Voice - Patient Interview: Misaki Aso. In the small office to the right of the previous stones.
Blue Spirit Stone x10. Continue by heading through the other door. On the other side you will find some stones on the small table to your left.
Hozuki Doll. Next to the table where you got the stones will be a large dresser. The doll can be found on the other end of this dresser.
Ghost (Specter) - Girls Running Down Corridor. Continue to the end of the hall and examine the final door on the right. Then, take a photo of it. Now, head back through the corridor to find this specter.
Ghost (Specter) - Girls Heading Somewhere. Continue backtracking. Once you get to the lift again you will see this specter running towards the hall on the left.
Hozuki Doll. Follow the girls through the door. In the next hall you will find the doll above the 3rd window on your left.
Ghost (Specter) - Girls Peeking Into Room. Continue down the hall and through the door. In the next hall you will find the girls again.
Hozuki Doll. After taking the photo of the previous specter, don’t move and you will see the doll in the box on the wall straight in front of you.
Hozuki Doll. Go through the door at the end of the hall. In the next room, look right to find the doll in the flowers.

Ghost (Revenant) - Girl in Coffin. In the same room as the previous collectibles, look through the camera until the circle turns blue while looking at the coffin. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.
Ghost (Specter) - Woman In Black By Coffin. After getting the key from the wheelchair, try to leave the room. The specter will appear behind you.
Ghost (Wraith) - “Me”. Walk towards the coffin to start this encounter.
File - Suicide Note. In the coffin.
Hozuki Doll. You will now have to go all the way back to the door that you took the photo of. Inside, look right to find the doll inside the container.
Lens - Grab Lens. On the shelf to the left of this room.

File - Medical File: Misaki Aso (1). Examine the puzzle on the dresser to the right in the same room.
Ghost (Wraith) - Shoji Katagiri & Shie Sonosaki / Shie Sonosaki (Blossomed). These ghosts will attack you as soon as you examine the puzzle.
Phase VI
Ghost (Specter) - Man Heading Towards Gallery. After examining and taking a photo of the door in front of you, go down the hall to the left to get this specter to appear in front of you.
Red Spirit Stone x5. Now turn around and go down the hall in the opposite direction as the ghost to find the stones in the corner.
Hozuki Doll. Now follow the specter in to the next room. Here, you will see a painting on the floor to your right. The doll can be found behind it.
Blue Spirit Stone x3. Go down the hall part of the room to find the stones in the corner.
Ghost (Specter) - Man Painting On The Floor. After taking a photo of the painting and getting the key from the gallery. Try to leave through the door that you used to get here. Then, follow the ghost through the other door to find this specter as soon as you go in.
File - Yuko Magaki’s Painting Notes (1). On the desk to the right of the previous specter.
Ghost (Specter) - Crawling Man. Go towards the other door in this room to get this specter to appear behind you.
Blue Spirit Stone x20. Can be found on the ground to the left of the painting on the floor.
Hozuki Doll. In the same room as the painting, head through the curtains to find the doll near the bath tub.
Ghost (Specter) - Girl Playing Piano. Exit the room with the painting on the floor through the other door. Then, use the key to open the door on the opposite side of the hall to your right (this is the door you tried to open at the very start). In this room, walk up to the piano to see the specter.
Hozuki Doll. Found inbetween the bed and the window in the same room as the piano.
Red Spirit Stone x5. If you are looking at the doll, turn around tu find the stones.
File - Ruka’s Diary (1). Examine the bed to find this file on the pillow.
Voice - Patient Interview: Ruka Yomotsuki. Head towrds the exit to start hearing a melody. Before doing anything else, look in the broken cabinet to find the voice file.
Blue Spirit Stone x3. Examine and take a photo of the music box. Then, head out to the hall and go right. Enter Misaki’s Room (first door on the right). Inside, you will find the stones at the foot of the bed.
Voice - Young Ruka’s Voice. Go to the back of the room for a cutscene to play. You will automatically get this voice file after the cutscene.
Voice - Young Ruka’s Secret. After the previous cutscene, examine the spirit radio to get the voice file.
Ghost (Wraith) - Shoji Katagiri. In the same room, examine the cradle to find the cog. You will then encounter Shoji again.
File - Ruka’s Diary (2). Return to Ruka’s Room and insert the cog in the music box. Solve the puzzle (solution in the screenshot below). Then, play the notes on the piano to get teleported to another place. You can now find the file on the bedside table.
Ghost (Specter) - Sayaka Looking In to Courtyard. Leave the room and go down the hall. Then, follow the ghost in to the room on the left. As soon as you step inside you can take a photo of the ghost

Ghost (Specter) - Girl Looking At Moon. Return to the hall and continue downstairs. Keep going until you reach a room with a table in the middle. Here your fillament will turn yellow. Look left to find the specter in the garden.
Ghost (Specter) - Sayaka Sitting In Chair. After picking up the photo from the table and returning to the normal world, Turn around and walk to towards the bed for this ghost to appear.
File - Ruka’s Diary (3). First, pick up the key from the draw on the floor in front of where the ghost was. Then, pick up the file from the same place.
Ghost (Specter) - Patient Hanging Head. Examine the mask on the bed and leave the bedroom. Go down the hall to the left until you see the ghost in the window in front of you.
File - Nurses’s Notes. Continue down the hall and go through the door at the end. In the next room you will find the file in front of you.
Ghost (Specter) - Nurse Standing In Station. Will appear in front of you as soon as you pick up the previous file.
Sayaka’s Diary (3). Enter the small “room” where the nurse ghost was standing. As soon as you enter look to your left to find this note.
Blue Spirit Stone x20. Found in the same cabinet above the previous file.
Red Spirit Stone x15. Leave the room where you got the previous collectible and use the lift to go down to 1F. Here run to the dining room to escape from the ghost. Once the screen goes back to the normal colours it’s safe to return to the lift room. Now, go down the hall where you saw the ghost. Here, examine the big metal door on your right and take a photo of it. Then, return to the lift area ti find the stones in the corner below the stairs.
Ghost (Specter) - Wandering Woman. From the stones, turn around and head through the door on your right. Go down the stairs to find this specter at the bottom.
Lens - Blow Lens. Enter the door on the left to find the lens on the ground near the desk to your right.
Blue Spirit Stone x30. On the shelves to the left.
Ghost (Specter) - Nurse On Watch Duty. Examine the hole in this same room to get the iron key. As soon as you pick it up you will see this ghost.
Ghost (Wraith) - Shie Sonosaki. Examine the desk where the ghost was sitting to start this battle.

Ghost (Wraith) - Takahisa Kozuki. Now, leave the room through the door on the east side. Then, examine the large wooden door at the end of the hall to start this fight (this can be a hard fight, so come well prepared).
Ghost (Revenant) - Woman Floating In Pool. Go to 1F and go to the pool area throug through the door in the northern hall. Go to the pool and look at the center of it through the camera until the circle turns blue. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.
Red Spirit Stone x10. Return to the hall and open the big metal door with the iron key. On the other side of the door, you will find the stones on your right.
Ghost (Wraith) - Yuzo Takemura / Yuzo Takemura (Blooming) & Katsuhito Kariya. Go down the stairs to start this double ghost encounter.
Hozuki Doll. After defeating the ghosts, look out the 2nd window on the right to find the doll on a tree branch.
Ghost (Wraith) - Natsuki Shiono / Natsuki Shiono (Blossomed). Go through the next door to enter the hospital area. Here, enter the first door to the left. Go through the first room to get to the room with beds in it. Pick up the sacred water from the bed to get a small cutscene. Now return to the entrance hall 2F. Here, there is a chance that you encounter Natsuki Shiono (again).
Blue Spirit Stones. Now, go down the stairs to get to Entrance Hall 1F. Here go to the shelves in the corner to find the stones.
Ghost (Specter) - Woman Sitting In Lift. Enter the lift in the entrance hall 1F to see this ghost.
Ghost (Specter) - Man Waiting For Lift. Ghost (Wraith). In the lift, press the very bottom button. Then, keep aiming the camera at the lift entrance as you go down to snap a shop of the ghost.
Hozuki Doll. Keep aiming the camera at the entrance of the lift to find the doll just after the specter.
Phase VII
Voice - Choshiro’s Investigaton Notes (4). On the ground in front of the shrine in the area you start in.
File - Scribbled Last Words. Go through the door near the shrine and follow the path to the end to find the note.
Ghost (Wraith) - Masanobu Fuchiki. This ghost will attack you after picking up the previous file.
Ghost (Wraith) - Masanobu Fuchiki, Michihiko Ikushima & Sato Mizunoe. Now, return to the shrine and continue to the lift to get attacked by these 3 ghosts.
Ghost (Specter) - Nurse In Front Of Mortuary. Take the lift to get to the next area. As soon as you walk out, the ghost will be on your left.
Blue Spirit Stones. On the ground near one of the beds where the save point is.
Ghost (Revenant) - Faceless Corpse. Now, follow the ghost through the door (the door near the lift). In this room take a photo of the mask on the bed to register this ghost.
Red Spirit Stone x15. On the bed near the door in the same room as the previous ghost.

Ghost (Specter) - Nurse Heading Somewhere. Follow the ghost through the door. On the other side, she will appear again.
Ghost (Wraith) - Sato Mizunoe, Masanobu Fuchiki & Kyoko Kitazume. Go down the hall to the left. At the end examine the control panel an take a photo of it. Then, return to where you found the previous specter and go down the other path until you encounter the 3 ghosts.
File - Construction Worker’s Logbook. From where you encounter the 3 ghosts, go through the hall to the east. This notebook will be at the end.
Hozuki Doll. In a wooden box at the end of the same hall as the previous collectible.
Ghost (Wraith) - Miyoshi Tsukigase. This ghost will attack you as you make your way back to the area where you encountered the other 3 ghosts.
File - Memo: “Special Treatment”. Once you are in the area where you encountered the 3 ghosts, head though the north hall and turn on the control panel to your right. Now, return to the control panel you took a photo of. Open it and go up the stairs. The file will be on the bed near the ghost that appears in the hall.

Ghost (Specter) - Yo Reading. Now follow the ghost through the door. As soon as you enter the ghost will appear in the corner.

Ghost (Specter) - Yo In Laboratory. Now, head down the stairs behind the cabinet in the same room. At the bottom, the ghosts will be around the bed at the bottom of these stairs.
File - Patient Observation Record. On the bed where the 3 ghosts are.
File - Haibara’s Assistant’s Notes (4). Head back upstairs and out to the hall. Then, go through the door to the north. In the next room the intercom will ring. Answer it and pick up this notebook that is found next to it.
Ghost (Wraith) - Sanae Hozuki / Sanae Hozuki (Blossomed). Leave this room through the other door. You will get attacked by this ghost in the next hall.
Ghost (Specter) - Nurse Heading Somewhere (2). Continue until you get to the Entrance Hall. As soon as you get there, this ghost will appear on your right.
Ghost (Specter) - Nurse Heading Somewhere (3). Follow the nurse out of the Entrance Hall. Then, follow this path until you get to the Reference Room (the room with all the boxes on shelves). As you walk through the Reference Room this ghost will appear on the other side of the shelves. Run around and take the photo.
File - Bundle Of Burned Letters. After following the ghost outside, walk forwards a few steps for a small scene to play. After the scene examine the incinerator to find the file.
File - “Film Incineration”. On the ground close to the incinerator.
Ghost (Revenant) - Hand In Garbage Bag. Look at the pile of bags through your camera until the circle turns blue. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.

Blue Spirit Stone x10. Follow the path until your item fillament turns blue. Here you can find the stones on one of the branches of the tree to your right.
Ghost (Specter) - Nurse Heading Somewhere (5). Take a few more steps up the main path for the specter to appear again.
Ghost (Specter) - Nurse Heading Somewhere (6). On the main path where the Tori gates are.
Lens - Chain Lens. As you walk through the Tori Gates you fillament will turn blue. The lens can be found on the ground to the left.
Ghost (Specter) - Nurse In front Of Grave. After the Tori Gates you will get to an area with a massive stone grave in the middle. The ghost can be found behind it.
Blue Spirit Stone x20. On the side of the grave.
File - Tsubaki Tono’s Diary. Found behind the grave where the specter was.
Hozuki Doll. On the south side of the grave.
Ghost (Wraith) - Masanobu Fuchiki, Michihiko Ikushima & Yuzo Takemura. As you try and leave the grave area, you will get attacked by these 3 wraiths. Unlike the previous encounters these 3 will respawn quite a few times. So be prepared for a long fight.
Ghost (Specter) - Man In Sink. Make your way back to the building. Go through rooms until you get to the “Operation Prep Room”. Here you will see the specter come out of a sink.
Voice - Unknown Voice (2). Once you get back to the Entrance Hall, answer the phone to get this voice note.
Ghost (Specter) - Yo Heading To New Building. Follow the ghost upstairs. Near the top you will see a cutscene. As soon as the cutscene ends, chase the ghost and take a photo of him.
Red Spirit Stone x10. Follow the ghost through the door. You will now be back at the ward. First head left to get to the Lif Lobby. Here you can find the stones under the stairs.
Blue Spirit Stone x3. Head through the south door to the Dining Hall. As soon as you enter look left to find the stones on the floor.
Voice - Choshiro’s Investigation Notes (3). After playing the film on the proyector in the dining hall, look behind the white screen to find the file.
Ghost (Wraith) - Michihiko Ikushima, Sato Mizunoe & Masanobu Fuchiki. As you make your way to the center of the room these 3 ghosts will attack you.
Ghost (Specter) - Nurse Entering Lift. Go back to the Lift Lobby. As soon as you enter the room you will see this ghost enter the lift.
Ghost (Specter) - Nurse In Front Of Station. Get in the lift and go to 3F. As soon as you exit the lift you will see the ghost.
File - 3F Nurse’s Logbook. Follow the ghost through the door. Inside, head in to the left part to find this note on your left.

Ghost (Specter) - Sayaka Gathering Belongings. Exit the previous room through the other door. Then, in the next hall enter room 308 (first door on the right). The specter will be near the bed.

Ghost (Specter) - Yo In Front Of Hidden Door. Return to the lift and go to 2F. As soon as you exit the lift, go to your left to get the specter to appear.
Blue Spirit Stones. In the small room under the stairs on 2F.
Ghost (Specter) - Ayako Squirming In Bed. Now head through the hall to the west. Go all the way to the end and enter room 207 (Ayako’s Room). Inside head towards the small hidden door for Ayako to appear on the bed.
Ghost (Wraith) - Ayako / Ayako (Blossomed). She will attack you after taking the photo of her in bed.
Note - Black Notebook (4). Go through the small door in Ayako’s room. Then head through the door on the other side of the bedroom. In the next room examine the jacket on the wall to your left to find the notebook.
Note - Black Notebook (5). In the same room, open the small box with the small key you got from Shoji. The note (and a sealed film) can be found inside.
Ghost (Wraith) - Yuko Magaki / Yuko Magaki (Blossomed). Return to 3F and enter room 309. The encounter will start as soon as you enter the room.

Ghost (Specter) - Girl In Black Leaving The Room. As soon as you take control of Misaki and take a few steps, this ghost will appear leaving the room you are in.
Voice - Conversation With Young Misaki. Examine the radio in the room you start in to get this voice file.

Ghost (Specter) - Girl Standing In Lift. Make your way to the lift lobby. As soon as you open the door to the lift you will see the ghost inside.
Ghost (Wraith) - Kyoko Kitazume / Kyoko Kitazume (Blossomed). Use the lift to go to 4F. This ghost will attack you as soon as you step out of the lift.
Hozuki Doll. On the chandelier in the area you fought the previous wraith.

Ghost (Specter) - Misaki And Girl In Corridor. Examine the intercom and take a photo of it to get the code (4467). Then, leave the room through the other door. As soon as you enter the hall you will see this ghost.
Ghost (Specter) - Nurse Blocking Door. Continue down the hall to find this specter in front of the door at the end.
Ghost (Wraith) - Shie Sonosaki / Shie Sonosaki (Blossomed). She will attack you after taking the photo of the previous specter.
File - 4F Isolation Room Report (3). Will spawn on the table in the same hall you encounter Shie.
Hozuki Doll. From the file, turn around and look up to find the doll sitting inbetween 2 metal bars.
Ghost (Specter) - Misaki And Girl Opening Door. Go through the door to the next hall. This specter will appear in front of the next door as you go towards it.
Hozuki Doll. In a hole in the ceiling in the same hall as you see the previous specter.
File - Haibara’s Assistant’s Notes (6). On the table just before you go through the next door.
Ghost (Specter) - Woman Holding Doll. Now, go through the door to the next room. After the cutscene the ghost will be on the bed.

File - Sakuya’s Diary (2). On the round table in the same room.
Ghost (Specter) - Younf Misaki Looking At Moon. Will appear on the other side of the window after reading the previous file.
Blue Spirit Stone x5. Go around to where the ghost was. The stones will be on the window.
Hozuki Doll. To the right of the stones you just picked up.
File - Misaki’s Diary. On the ground where the ghost was standing.
File - Sayuka’s Diary (3). Now, go to the corner of the room whith all the dolls. Here you will have to solve the puzzle (just place the numbers in the correct order so they look like the picture at the top right of the screen.
Ghost (Specter) - Girl In Black Staring At Misaki. After solving the puzzle, walk towards the bed until this ghost appears.
Ghost (Specter) - Patient About To Jump Off Roof. After getting close to the previous specter a cutscene will play. As soon as the cutscene ends, this ghost will be in front of you.
Woman’s Suicide Note. Interact with the ghost to get this file.
Upgrade - Hear. Turn around to find the upgrade inbetween the 2 benches.
Hozuki Doll. On the ground to the right of the benches.

Hozuki Doll. Go through the door to the right of the red stones. In this room you will find the doll behind a broken mirror on the right.
Blue Spirit Stone x5. Do through the next door and down the stairs to find a save point. Near the save point go through the door with curtains. Inside you will find the stones.
Hozuki Doll. In the same room as the stones, go to the end to find a fan on the wall. Here, look left to find the doll.
Hozuki Doll. Now, go past the save point and in to the next room. Here you will find the doll behind the wheelchair.
File - Patient’s Note. On the floor inbetween the beds in the same room as the previous doll.
Ghost (Specter) - Man Standing In Front Of Room. Go through the next door. You will see the specter in front of you after taking a few steps.
File - Fragment Of Unsent Letter. Follow the ghost in to the room on the left. The file can be found on the bed to the right.
Hozuki Doll. On the same bed as the previous file.
Ghost (Wraith) - Takashi Aiba / Takashi Aiba (Blossomed) & Iori Aiba. Try to leave the room for this encounter to start.
Ghost (Specter) - Director Beconing Misaki. Leave the room and go left for this ghost to appear.
Hozuki Doll. To your left you will have a small area with glass walls. The doll can be found in the bucket inside this area.
Ghost (Specter) - Director Descending Stairs. Go through the door to the next area. You will then see the specter going down the stairs.
Ghost (Specter) - Director heading To Exam Room. Follow the director down the stairs to see him again at the bottom.
Ghost (Wraith) - Himiko Kiriya. In the entrance hall, save your game and head out the door to the north to go outside. Here you will be attacked by this ghost.
Red Spirit Stone x10. On the graound near the gate in the outside area.
Hozuki Doll. On the wooden beams above the well in the same area.
Ghost (Revenant) - Woman Who Fell From Roof. Still outside, look through the camera and aim to the left of the left most kanji above the entrance. As soon as the circle turns blue, take the photo.
Ghost (Specter) - Director Entering Door. Now return to the entrance hall and follow the director (take the hall to the south west of the entrance hall). Continue on this path until you get to the hall in the Medical File Room. Here you will see the director again.
Ghost (Specter) - Director Entering Office. Follow the previous specter through the door. In this area you will see him again when going down the hall.
Blue Spirit Stone x10. Ignore the director for now and head down the stairs in front of you. At the bottom you will find the stones on the floor.
Ghost (Revenant) - Hanged Woman. Continue through the hall at the bottom of the stairs. When the path splits, take the door on the left. In the next area go right until you see a peephole on the wall to your left. Aim the camera at it to get this revenant.
Ghost (Revenant) - Eyes Looking In Window. Now, return upstairs to the room the director was entering. Inside aim your camera to the right of the cabinet (the secret door) to find this revenant.
File - Medical File: Misaki Aso (2). On the table in the middle of the same room.

Ghost (Specter) - Director Haibara And Misaki. Examine the back of the directors desk to open the secret door to the underground. Go down to find these ghosts in the middle of the room.
Ghost (Wraith) - Himiko Kiriya / Himiko Kiriya (Blossomed). This encounter will start automatically after taking the photo of the previous specter.
File - Medical File: Misaki Aso (3). On the bed in the middle of the room after the wraith encounter.
Ghost (Specter) - Girl Watching Misaki. Will appear to your right as soon as you pick up the previous file.
Lens - Zero Lens. On the floor near the bed in the same room.
Ghost (Specter) - Girl Heading Underground. Follow the ghost down the stairs. She will appear again at the bottom.
Ghost (Specter) - Misaki Being Led. After going through the small door at the end, take a few steps to see this ghost.
Ghost (Wraith) - Michihiko Ikushima, Sato Mizunoe & Masanobu Fuchiki. You will face these 3 ghosts in the same room as the previous specter.
File - “Legend Of The Moonglow Well”. On the altar in the same area.
Ghost (Specter) - Girl Heading Somewhere. After getting the previous file a cutscene will play. After the cutscene the specter will be in front of you.
Ghost (Wraith) - Sakuya Haibara. Go through the door to face the final wraith of the chapter.
Phase IX
Ghost (Wraith) - Masanobu Fuchiki & Michihiko Ikushima. Walk towards the shrine at the start of the level to start this encounter.
Ghost (Specter) - Trembling Islander. Go through the door to the right of the shrine, past the save point and through the hole. In the next room, head right. Then, at the altar, head right to find the ghost at the dead end.
File - Note Left By Fisherman. Found at the end, where the specter was.
Ghost (Specter) - Frightened Islander. Turn around and head straight until you get to another dead end. The specter can be found at the end.
Ghost (Wraith) - Sato Mizunoe. Go to where the specter was to encounter this wraith.
Hozuki Doll. Return to the fork in the path after you crawled through the hole. This time take the other path (the one to the north). Next, take a right, then another right before going up the stairs. Next, go left and then right (up the stairs). Go to the balcony at the end to get a cutscene. Then, turn around and go right. After taking a few steps, turn around to find the doll above you.
Blue Spirit Stone x20. Turn around and continue down this path. After going down the stairs, go left and left again. Keep walking until you get a cutscene. After the cutscene look to your left to find the stones.
Blue Spirit Stone x20. Now, go down the stairs. At the bottom the ghost will attack you, so turn around and run back up the stairs until the screen goes back to normal. Then, go back down to where the ghost was to find the stones on the wall to your right.
File - “The Rogetsu Primeval Passage”. Continue down the next set of stairs. You will see a shrine in front of you. Here, go left to find the file at the end.
Ghost (Wraith) - Masanobu Fuchiki / Masanobu Fuchiki (Blossomed). This wraith will attack you after reading the previous file.
Red Spirit Stone x5. Continue down the path to the right of the shrine. At the end of this tunnel you will see the stones on the ground to your left.
File - Note Left By Islander. Continue straight down the tunnel to the end. You will find the file on the ground.
Ghost (Wraith) - Masanobu Fuchiki. He will attack you after reading the previous file.
Ghost (Specter) - Woman In Primal Tunnel. Turn aroun (from where you got the file) and go through the tunnel on the left. Continue until this ghost appears.
Ghost (Wraith) - Sato Mizunoe. Walk towards the specter to start this encounter.
Lens - Zero Lens. Continue through the tunnel until you encounter the ghost yuu have to run away from. After doing that, return to where the ghost was and go right. At the statue go right and down the stairs to find the lens at the end.
Red Spirit Stone x5. Turn around and walk past the statue. At the end you will find the stones.
File - Rogetsu Kagura Chronicle. Now return to the statue, go right and right again. Follow this path until you get to the top of the long stairs going up. At the top turn around to find the file on the right side of the altar.
Red Spirit Stone x20. Now, go to the door to find the stones on the right side of it.
File - “To Rigetsu Kagura Visitors”. Go through the door and up the stairs until you see a path blocked by some wooden beams. The file can be found where the beams are.
Ghost (Specter) - Organs Entering Kagura Stage. These ghosts will appear on the other side of the wooden beams after reading the file.
Ghost (Wraith) - Organ (Strings), Organ (Drum), Organ (Flute), Organ (Bell), Organ (Vocal) & Vessel. Continue up the stairs, past the save point and through the door on the right side of the hall. You will now have these 6 wraiths on the dance stange.
Hozuki Doll. You will now have to leave the stage by going up the stairs on the other side. Before you head up the wooden stairs, look behind them to find the doll.
Blue Spirit Stone x30. Now head up the stairs and through the door. Here go right to the end to find the stones (if you go left you can find some Type-00 film).
File - “History Of The Rogetsu Antechamber”. Now go down the stairs until a cutscene plays. Then, follow the ghosts down. In the area at the bottom you can find the file near the center of the room.
Ghost (Revenant) - Vessel And Organs In Trance. Go to the shrine and turn around. Aim your camera at the center of the room to register the revenant.
Hozuki Doll. In the corner to your left in the same room.
Ghost (Specter) - Soya Facing Away. Go back up the first set of stairs and go down the hall. Keep going until a ghost appears in front of you.
Ghost (Wraith) - Kyoko Kitazuma. In this same hall you will see a lamp on the right. Examine it to start this encounter.
Hozuki Doll. On the other side of the wood at the end of the hall.
Hozuki Doll. Go through the door near the doll. In this hall look through the gaps in the first window to your left to find the doll behind the tree.
Ghost (Specter) - Girl In Pain On Altar. Continue down the hall until you see a door on your left. Inside the girl will be on the altar in the middle.
File - “Secrets Of The Shugen Temple”. On the shrine at the back of the room.
Hozuki Doll. On the right side of the shrine.
Ghost (Specter) - Soya Returning From Temple. Examine the door on the left of the shrine. Then, leave through the door you used to get here. Go down the hall on your left until you see the specter.

File - Yuko Magaki’s Painting Notes (2). On the table in the room where you start.
Ghost (Specter) - Yo Entering Gallery. After taking a photo of the painting on the floor, leave the room to find this specter to your left.
Ghost (Specter) - Yo Looking At Picture. Follow the ghost in to the next room to find him at the back looking at the picture.
File - Black Notebook (3). Found where the previous specter was.
Ghost (Wraith) - Kageri Sendo / Kageri Sendo (Blooming). Examine the 2 paintings where the specter was. Then, turn around and go past the wheelchair to encounter Kageri Sendo.
File - Kageri Sendo’s Letter (3). Obtained after defeating Kageri Sendo.
Upgrade - Acquire. Make your way to room 311 (Kageri’s Room) on this same floor. Inside, head through the curtains to find the upgrade on the coffin.
File - 4F Isolation Room Report (4). Next, head to the lift hall and take the lift to 4F. Here, go right and answer the intercom. The file can then be foun in the same small room as the intercom.
Voice - Yo Haibara’s Tape. Now go through the hall to room 412. The voice file can be found on the floor near the bed.
File - Sakuya’s Diary (4). Obtained at the same time as the tape.
Ghost (Wraith) - Shoji Katagiri / Shoji Katagiri (Blossomed) & Shie Sonosaki. These 2 ghosts will attack you after reading the files.
File - Haibara’s Assistant’s Notes (7). Obtained after defeating the ghosts.
Ghosts (Wraith) - Shie Sonosaki & Fuyuko Shiratsuki. Return to the lift for these 2 wraiths to attack you.
File - On-Duty Nurse’s Notes (2). Obtained after defeating the 2 wraiths.
Ghost (Revenant) - Ayako Blossoming. Return to the lift and go to 2F. Then, make your way to Ayako’s room. Enter through the small door to get to the main bed. Here take a photo of the pillows to register this revenant.
Blue Spirit Stone. Go through the other door in the bedroom. In the next room look down to your right to find the stone.
Ghost (Wraith) - Woman In Red. Return to the lift and go to 1F. Here go through the northern hall and take the 1st door on the left to get to the garden with the pool. Here, walk to the pool to find this wraith.
Voice - Choshiro’s Investigation Notes (6). Go down the stairs in the pool area. Once you get to the little room with the metal grates on the floor below you, the tape will be on the shelf.
File - On-Duty Nurse’s Notes (3). Continue out the other door. Then, go to the other end of the hall and enter theSwitchboard Room. Here, examine the radio for a cutscene to play. Then, pick up the file on the left of the radio.
Ghost (Revenant) - Woman Under Floor. Take a photo of the open hole on the floor in the middle of the room (if it’s not open, leave the room and come back in).
Hozuki Doll. Continue by going out the door on the other side. Go to the end of this area and use the key to open the wooden door. In the next room, head right and look through the locked gate to see the doll on the other side.
Blue Spirit Stone. On the ground near the locked door.
File - Note By Court Guardian. On the shrine in the same area.
Red Spirit Stone x15. To the right of the door you used to get here.
File - Note From Someone. On the dresser in the small room with the bed to the left of the shrine. You can also find the Black Mask Fragment (Right) on the bed.
Ghost (Revenant) - Man With Face Cut Off. Look at the end of the bed in the locked room and take a photo to get this revenant. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.
Ghost (Specter) - Yo Heading To Old Building. You will now have to return to the lift hall. Then, go through the hall to the north. Here you will see this specter.
Ghost (Wraith) - Himiko Kiriya & Yoriko Sonohara. Follow the ghost through the door to encounter these 2 wraiths.
Voice - Yo Haibara’s Voice. Continue down this path to get to the hospital. Then, go down the stairs to get to Entrance Hall 1F. Answer the phone to get this voice.
File - Director Haibara’s Notes (3). Now go down the hall to the right and make your way to the director’s office to find this note.
Ghost (Wraith) - Shigeto Haibara, Michihiko Ikushima, Masanobu Fuchiki. Next, return to the entrance hall and enter the lift and press the bottom most button. On this floor you will encounter these 3 ghosts.
Ghost (Specter) - Yo Escaping From Choshiro. Get back in the lift. This will start moving by it’s self. As soon as it stops this specter will run across in front of you.
Ghost (Specter) - Yo Being Chased. As soon as you leave the lift, go through the door on the right. Then, go to the door at the end of the hall. In this next room you will see the ghost.
Ghost (Specter) - Yo Heading To Roof. Follow the ghost through the door. You will see him going up the stairs just past the save point.
Blue Spirit Stone x30. Head in to the laundry room on the right. In this room you will find the stone son the floor.
Ghost (Wraith) - Asagi Hizuki /Asagi Hizuki (Blossomed). From the save point, go back through the door to the previous hall. Then return to the save point area. Keep doing this until Asagi appears. This seems like one of the only places she can blossom. After this, follow the ghost up to the roof to finish the chapter.
Phase XI
Ghost (Specter) - Soya Walking Down Corridor. In the room you start in, head towards the door. Before going through, head down the hall to your right until the specter appears.
Hozuki Doll. Now go through the door and look up to your right to find the doll on the shelf.
File - “Lineage Of The House Of Yomotsuki”. Now, head out the other door and go through the door on the other side of the hall. The file will be on the table.
Hozuki Doll. In the cabinet to your left.
Ghost (Specter) - Soya Returning To His Room. Now, go back to the hall and go right. You will see this specter as you approach the corner.
Blue Spirit Stone x30. Go to the end of the hall to find the stones.
Ghost (Wraith) - Takahisa Kozuki. Examine and take a photo of the door on your left. Then, return to the room where you picked up the file. To the right of where the file was you will see a mask. Examine it to get attacked by this ghost.
Blue Spirit Stone x10. Leave the room and go down the hall to your right. Then, go through the door on the right. Here, save you game and pick up the blue item in front of you.
Voice - Call From Sayaka. Answer the phone next to the stones.
Hozuki Doll. Now head towards the door with the mask on it. Before you open it, look at the cabinet to the right to find the doll and some Type-90 film.
Ghost (Specter) - Sayaka. Examine the mask on the door for the ghost to appear behind you.
Ghost (Specter) - Sayaka Standing. Continue down the hall to the west for this ghost to appear.
Ghost (Specter) - Ruka In Garden. Go through the door on the right to get to the living room. As soon as you enter, the ghost will be in the garden to your right.
Hozuki Doll. In a half open cabinet in this same room.
File - “Secrets Of The House Of Yomotsuki”. On the shrine in the next room.
Hozuki Doll. Go through the door near the altar. Then go down the hall until you see a round window on your right (just before some stairs). The doll will be outside on the tree.
Ghost (Specter) - Sayaka & Ruka At Piano. Now, go up the stairs and go to the door at the end of the hall. The ghosts will appear at the piano as soon as you enter the room.
Hozuki Doll. At the foot of the bed in the same room.
File - Ruka’s Diary (4). On the table in the middle of the same room.
Blue Spirit Stone x20. Play the piano and leave the room. Go down the hall, go around the corner and enter the door on the left. Once inside the stones can be found near the mirror.
File - Letter To Soya. On the bed in the same room.
Hozuki Doll. Inbetween the wall and the head of the bed in the same room.
File - “Secrets Of The Guardians”. Examine the white door in this same room to get this file (and a mirror).
Ghost (Wraith) - Takahisa Kozuki. Place the mirror on the stand opposite the bed and take a photo of the wall near the bed. Then, examine the sealed mask on the shelf in this same room. You will now have to make your way back to the sealed door you took a photo of at the start of the chapter. On the way you will get attacked by this ghost in the entryway corridor.
Ghost (Specter) - Soya Unrolling Scroll. Open the sealed door and take a few steps inside to get the ghost to appear.
Hozuki Doll. Turn around and look right to find the doll in the vase.
File - Soya’s Notes (2). Examine the mask where the ghost was. You will then find this file in the cabinet in the south east corner of the room.
File - Soya’s Notes (1). On the books to the left of where the specter was.
Ghost (Specter) - Islanders In Parlor. Leave the room and make your way down the hall until the ghosts appear in the room to your right.
Red Spirit Crystal x15. Go through the room with the ghosts to get to the courtyard (the area where you started). Here, go through the double door to leave the house. On the other side you will see the stones on the ground.
Ghost (Specter) - Organs Heading To Kagura. Continue through the hall to get these ghosts to appear.
Ghost (Specter) - Ancient Islanders. Go through the next door to find the specter in the next hall.
Ghost (Wraith) - Organ x5. Follow the specters to the next room to face the 5 ghosts again.
Red Spirit Stone x5. Solve the puzzle on the altar (solution below) to reveal a door. GO through the door, past the save point and down the first set of stairs to find the stones.
Ghost (Specter) - Yo And Young Ruka (1). Continue down the stairs to encounter the specters.
Blue Spirit Stone x30. Continue down 2 more sets of stairs. At the bottom of the 2nd you will find the stones.
Ghost (Specter) - Yo And Young Ruka (2). You will see them going down the next set of stairs.
Ghost (Specter) - Yo And Young Ruka (3). You will see the specters again once you get to a hall with lamps.
Ghost (Specter) - Yo And Young Ruka (4). Continue around the corner to find the ghosts again.

Hozuki Doll. Before you go through the wooden door, look up to your right to see it in the wooden structure.
Hozuki Doll. Go through the wooden door and look up to your right (it will be in the same place as the one on the other side of the door).
Ghost (Wraith) - Sato Mizunoe & Miyoshi Tsukigase. Go to the center of the area. Then, run allk the way back to the save point to lose the ghost that is chasing you. Finally, return to the circular room where the ghost started chasing you to encounter these ghosts.
Ghost (Specter) - Regretful Soya. Return to the save point and backtrack towards the house you started in. On the way back you will encounter this specter.
Ghost (Specter) - Soya Entering Artisan Room. Follow the previous specter and go through the door on the right. Soon after you will see this specter going towards the door on the left (you will have to be quick with this one).
Ghost (Specter) - Ruka Heading To Meet Soya. Follow the specter in to the next room to see this one.
Ghost (Revenant) - Enshrined Facial Skin. Look through the camera at the shrine until the circle turns blue. TO FIND THIS REVENANT YOU WILL NEED THE “SENSE” UPGRADE. THIS IS OBTAINED AFTER FINISHING THE GAME AND HAVING FOUND AT LEAST 100 GHOSTS.

Ghost (Wraith) - Soya Yomotsuki. Now go back and continue up the center stairs to encounter the ghost for the last time.
File - Soya’s Notes (6). Obtained after killing Soya.
Hozuki Doll. Continue up the middle stairs until you get to a dead end. The doll will be on the stairs in front of you.
Blue Spirit Stone x10. Now go down the stairs until you can go to the side. The stones will be on the window.
Ghost (Revenant) - Guardian Maidens. From the save point, head down the stairs to the dead end. Here look through the camera at the central stairs. As soon as the circle turns blue take the photo.
Ghost (Specter) - Eclipse Sayuka At Lighthouse. From the save point, continue up teh stairs and go through the door. At the top of the lighthouse you will find the ghost.
Ghost (Specter) - Sayaka Watching Over Ruka. Continue walking forwards to get this specter to appear.
Blue Spirit Stone x15. On the railing to your left.
File - “Legend Of The Moonsong”. Go to where the ghost was to get this file.
Hozuki Doll. Found on the other side of the railing to your left.
Ghost (Wraith) - Eclipse Sakuya. Climb the ladder to face the final ghost of the game!
Hozuki Doll. Found on the right side of the shrine.
POST-GAME NOTES:
TO UPGRADE THE CAMERA TO MAX LEVEL WITH EVERY CHARACTER YOU WILL HAVE TO PLAY THROUGH NEW GAME+ AND PICK UP THE STONES AGAIN (THEY WILL RESPAWN IN THE SAME LOCATIONS).
IF YOU ARE STILL MISSING A FILE, GO IN TO THE IN-GAME PHOTO MODE (THE TUTORIAL MAY COUNT AS A COLLECTIBLE FILE).
YOU WILL ALSO HAVE TO PLAY THROUGH NEW GAME+ TO GET THE HIDDEN REVENANTS WITH THE “SENSE” TOOL (LOCATIONS ARE MENTIONED IN THE GUIDE).
