To get the “Not Even My Final Form” trophy / achievement you will have to kill a mutator before it transforms. You can find a Mutator Hot Spot in the house to the southeast of Beverly Hills (only after finishing the game). The mutator will spawn in a random location inside. You will have to kill it before it detects you. The best way to do this is to use an upgraded sniper from up close and then melee it with a heavy weapon if it’s still alive. Once the mutator is dead you will get the trophy.
